Form 5 Application For Registration of One Dog in Obedience & Working Trials Register
Form 6 Transfer of Registered Dog
Form 7 Change of Name (Other than the addition of an Affix)
Form 8 Change of Name (To be used for the addition of an Affix)
Form 9 Application for Registration of a Border Collie which:
a) Is already registered with the ISDS
b) Has both parents registered with the ISDS or
c) Has the dam registered with the ISDS and the sire with The Kennel Club.
Form 10 Application for the Registration of an Affix
Form 11 Application to Vary the Ownership of an Affix
Form 13 Form for the Re-registration of a Dog
